Cancellation Policy

All cancellations must be submitted in writing (email or by US Mail).

Qualifying Examination
  • Prior to January 15, 2011 = $75 administrative fee.
  • January 16, 2011 through April 15, 2011 = $100 administrative fee
  • After April 16, 2011 = Attempts to cancel or reschedule after the deadline will result in forfeiture of the entire registration fee
  • Failure to appear for the scheduled testing appointment = Candidate will be considered a “No Show” and will forfeit the entire $700 registration fee (see Section III.D).

Oral Clinical Examination

September 20-24, 2010

  • Cancel prior to or on May 17, 2010 - refund less $100 administrative fee
  • Cancel May 18-June 30, 2010 - 1/2 examination fee will be refunded
  • Cancel on or after July 1, 2010 - no refund
