ALL cancellations must be in writing.
Qualifying Examination -
All cancellations are assessed a $75 administrative fee.
If a candidate fails to cancel or reschedule their testing session up to five (5) business days in advance of their testing appointment, the candidate will forfeit $350 of the $700 registration fee.
If a candidate fails to appear for their testing appointment they will be considered a 'No Show' or attempts to cancel or reschedule after the five (5) business day deadline, the candidate will forfeit the entire $700 registration fee.
Oral Clinical Examination

Cancel July 23-August 21, 2008 - 1/2 examination fee will be refunded
Cancel on or after August 22, 2008 - no refund
