Verification of Board Status

The American Board of Pediatric Dentistry requires a $25 fee per verification as well as having the request in writing with a copy of the general release of information that has been signed by the pediatric dentist that is being reviewed.  The verification request and payment need to be directed to the address above; we do accept MasterCard or Visa. If a credit card is being use you can fax to our office at 319-341-9499.
